GGHA Representative Coaching Application 26-27 (Georgina Girls Hockey Association)

I, authorize Georgina Girls Hockey Association (GGHA) to perform reference checks and collect personal information appropriate to the position applied for. Upon request, I also agree to provide to the GGHA a police criminal check as part of their review process. If my application for the coaching position is successful, I agree to abide by all GGHA (and its governing bodies OWHA, OHF, Hockey Canada) rules, regulations, policies, guidelines, bylaws and Code of Conduct. I further understand and agree that as Head Coach I bear ultimate responsibility for all coaching staff conduct or lack of performance in his or her duties. I also accept responsibility for all player conduct on and off the ice and will ensure that the team will display the highest level of sportsmanship in representing the Georgina Golden Hawks, and as ambassadors to women’s hockey. I also agree to attend or participate in any and all association meetings deemed to be mandatory, such as but not limited to, pre-season meetings, Year-End awards ceremonies.
