Self Assessment OWHA Health Screening
The attached Self Assessment must be completed before every event and will be confirmed by team Coivd Administrator.  OWHA is working with a provider developing an App for this and we will use as soon as available.  This must be completed for all players, team staff, volunteers and for U11 and U9 one spectator per player.
