If you played with Georgina this past year, you DO NOT need to fill out this form to try out in Georgina. If you wish to try out elsewhere, you need to complete the form. All Permission to Skate (PTS) forms will be sent via email. Players are required to complete one form for each Association they wish to try out for. Once this form is submitted, a signed form will be emailed to you at the email address provided. Please do not email requests directly.
